Year-round Newborn Enrollment

(Birth to 1st Birthday)

Background

• You may enroll a child from the time they are born to their 1st birthday any time of year (no enrollment period limitations).

• The following must be included in your account packet:  enrollment form, copy of the birth certificate, enrollment fee and payment.  Enrollment forms received without a birth certificate copy will not be processed, and may be returned.

   

Steps to complete your enrollment

• Complete the form, print and sign it, and attach a copy of the child's birth certificate.
